It is the responsibility of the aircraft owner or operator to submit this Maintenance Claim Form with all supporting documentation.
Maintenance Discount Conditions
An aircraft is entitled to the Maintenance Discount listed in the Conditions of Use for a maximum of 3 months if it meets all the following conditions:

- The aircraft is undergoing maintenance by a Licensed Aircraft Maintenance Engineer (LAME)
- This form is verified and signed by a LAME within 5 days of commencement and 14 days after completion of maintenance
- Where maintenance exceeds 14 days, a certified copy of the Maintenance Release form supports this claim
- Documentation evidence of maintenance commencement (for example technical log entry or maintenance log entry)
- Documentation evidence of the maintenance release (for example Maintenance Release or Certificate of Release to Service)
- Bankstown Airport Pty Limited approves the Maintenance Claim Form
